1. USETPA contracts are available to all Public and Private K-12 Schools, Libraries and Government Entities
  2. Members/Subscribers are never charged a fee to use a USETPA Contract
  3. Membership does not obligate the member/subscriber to use a USETPA contract
  4. Erate specific: Optional (highly remmended but required to use our contracts for our Erate 470's for purchases)
  5. If you wish to register for the potential use of USETPA Erate 470's, then the authorization/membership form optionally authorizes the USETPA to conduct bids through the Form 470 on behalf of the subscriber by checking the appropriate box. Bids will be conducted for all category one and category two services. More details are on the actual membership/subscriber form. You are under no obligation to ever use this 470, but it is a great backup if you forgot something. Be sure to include your BEN (Billed Entity Number).

This form may be completed by one of three methods.
